# APEX Health Services

> APEX Health Services is an Illinois in-home care provider serving Chicago and northern Illinois. It offers home health, hospice, palliative care, transitional care, chronic disease management, non-medical home care and private duty care in patients' homes, nursing homes and assisted living.

APEX Health Services is the shared brand of three affiliated Illinois companies: APEX Home Health Care, Inc. (home health), APEX Hospice and Palliative Care, Inc. (hospice, palliative and transitional care) and Apex Home Care, Inc. (non-medical home care); private duty care is offered as APEX Private Duty. It was founded in 2008 by M. Mannan, Zameer Baig and Aqil Rab as a home care agency licensed by the Illinois Department of Public Health, added hospice and palliative care in 2011, and holds CHAP accreditation for hospice care since 2012. It serves Cook, DuPage, Kane, Kendall, Lake, McHenry and Will counties, for hospice also Boone, DeKalb, Grundy, Kankakee, Lee and Winnebago, and for palliative and transitional care also LaSalle, Livingston and Ogle. Home health is physician-ordered and covered by Medicare and most insurance plans; hospice and palliative care accept Medicare, Illinois Medicaid, VA, TRICARE and listed commercial plans; non-medical home care is funded through the Illinois Department on Aging Community Care Program and DORS; private duty is private pay or long-term care insurance. Care is given where the patient lives, not at the offices, and the home care lines are non-medical only.
