Primary Focus: A single-provider continuum of in-home care. APEX uses "In-Home Care Services" as the umbrella term, with a firm distinction between Home Health Care (medical services in the home) and Home Care (non-medical support). Patients can move between lines — for example, transitional care can hand off to home health, palliative, or hospice — without changing providers.
