Improving Access and Timely Care for Vulnerable Patients Referred to Home Health via Telehealth Physician Support** Apex Health Services partnered with Sound Physicians to develop a scalable, evidence-based program leveraging telehealth waivers during the COVID-19 Public Health Emergency.
