Levels of Hospice Care
Routine Home Care
Routine home care is delivered at the patient's home with scheduled visits by hospice team members, according to the patient's needs. Team support is available 24/7 via phone for emergencies, with nurses and counselors ready to visit in case of a crisis. All necessary medications, medical equipment, and supplies are provided by hospice.
General Inpatient Care
For patients with severe symptoms like uncontrolled pain or respiratory distress, care is provided in hospitals, skilled nursing facilities, or hospice inpatient units. This level of care is determined by the hospice team and arranged accordingly.
Respite Care
To offer relief to family caregivers, Respite Care provides up to five days of care in a nursing facility, hospital, or hospice unit. This service is pre-arranged by hospice under Medicare and Medicaid.
Continuous Home Care
During critical periods, Continuous Home Care offers a minimum of eight hours and up to 24 hours of daily care at home to achieve palliation and management of acute medical symptoms.
