    Chicagoland Premier In-Home Hospice Care

APEX offers comprehensive hospice care services in the familiar environment of a patient’s home, assisted living facility, or nursing home. Our goal is to alleviate physical and emotional suffering for both patients and their families during this profound life experience.

    What Is Hospice Care?

Hospice care focuses on comfort rather than curing a terminal illness. It offers medical care, emotional support, and spiritual resources for individuals in their final months, ensuring their comfort and dignity. This care extends to caregivers, ensuring they have the support and knowledge needed during this time.

A common misconception is that hospice care necessitates a visit to a specialized facility. However, APEX offers the convenience of delivering hospice care right where your beloved family member resides. Typically, this means their own home or an apartment, although it can also extend to nursing homes or assisted living facilities.

Our Experienced and Compassionate Team Offers:

    • Regular home visits from a hospice interdisciplinary team that includes a hospice nurse and physician.

    • Medication management for symptom relief.

    • Support with medical equipment for comfort.

    • Emotional and spiritual support for patients.

    • Specialized veterans hospice care.

    Caregivers Can Expect:

    • Guidance and education on patient care.

    • 24/7 access to hospice nursing support.

    • Emotional and spiritual care for families.

    • Assistance with family matters and respite care.

    • Anticipatory grief counseling and support for coping with loss.

What Are the Benefits of Hospice Care?

Hospice care offers comprehensive support at home, easing the challenges of a terminal illness. It focuses on:

    • Enhancing physical comfort by alleviating symptoms such as pain and nausea.

    • Providing emotional support to reduce worry and address personal or family concerns.

This approach allows you and your loved one to cherish your time together in a familiar, comforting environment. Hospice care supports a dignified and fulfilling experience for your loved one at home.

    Levels of Hospice Care

    Collaboration with Apex HHC

    Routine Home Care

Routine home care is delivered at the patient's home with scheduled visits by hospice team members, according to the patient's needs. Team support is available 24/7 via phone for emergencies, with nurses and counselors ready to visit in case of a crisis. All necessary medications, medical equipment, and supplies are provided by hospice.

    General Inpatient Care

For patients with severe symptoms like uncontrolled pain or respiratory distress, care is provided in hospitals, skilled nursing facilities, or hospice inpatient units. This level of care is determined by the hospice team and arranged accordingly.

    Respite Care

To offer relief to family caregivers, Respite Care provides up to five days of care in a nursing facility, hospital, or hospice unit. This service is pre-arranged by hospice under Medicare and Medicaid.

    Continuous Home Care

During critical periods, Continuous Home Care offers a minimum of eight hours and up to 24 hours of daily care at home to achieve palliation and management of acute medical symptoms.

    Navigating Our Process

    Referral and Consultation

Upon receiving a doctor's referral for hospice care, we initiate our process by meeting with the patient's family. In this meeting, we:

    • Clearly explain the scope and benefits of hospice care.
    • Secure hospice consent from the patient or legal representative.

    Admission and Care Coordination

After obtaining consent, we admit the patient into our program and organize essential services, including:

    • Arranging required medications, medical supplies, and equipment.
    • Assigning a dedicated care team, including a nurse, hospice aide, social worker, and spiritual counselor, ensuring holistic care for the patient.

Our goal is to provide comprehensive care, focusing on both the medical and emotional needs of the patient.

How Is Hospice Care Paid For?

Home hospice care is financed through Medicare, Medicaid, the Department of Veterans Affairs, and most private insurers, with coverage beginning immediately upon admission to the program.

Committed to accessible care, we do not deny hospice services based on financial constraints, adhering to ethical and regulatory standards. Our focus remains on providing essential hospice support to every patient, regardless of financial status.

When do you need hospice care?

Consider hospice care if managing symptoms becomes challenging, frequent medical intervention is needed, or if treatments are less effective. Early hospice care can significantly improve the quality of life for your loved one and provide much-needed relief and support for you as a caregiver.

Who qualifies for in-home hospice care?

In-home hospice care is intended for patients with a life-limiting illness, typically with a prognosis of six months or less. It's for those prioritizing comfort over curative treatment, needing assistance with daily living due to their illness, and preferring to spend their final days at home.

Are APEX caregivers screened and trained?

Our caregivers undergo thorough background checks and specialized hospice care training. Their ongoing education ensures adherence to the latest care practices. Regular skill assessments and a focus on empathy and compassion are key. This rigorous process ensures caregivers are not only skilled but also deeply empathetic in providing end-of-life care.
