Our Inpatient Unit

Located at 4390 Illinois-71 Wing 500, Oswego, IL 60543, our Hospice Inpatient Unit is currently welcoming patients and their families. It’s a thoughtfully designed facility that creates a peaceful, home-like atmosphere where families can gather, rest, and spend precious time together during this important journey.

Our accommodations include:

  • 14 comfortable beds across 8 carefully appointed rooms

  • 2 private rooms for complete privacy and tranquility

  • 6 semi-private rooms with flexibility for family involvement

  • Private bathroom in every room for dignity and convenience

Our team at APEX Hospice and Palliative Care, Inc., believes that love and connection are fundamental to comfort care. Our comprehensive visiting policy ensures that families stay at the center of their loved one's care experience.

Open Door Policy

We welcome families around the clock with no restrictions on visiting hours. Patients and their families have complete control over their visiting experience — they choose who visits, when visits occur, and how long they last. Visiting features include:

  • 24/7 visitation is encouraged and permitted

  • Overnight accommodations are provided in patient rooms

  • Children are welcome when accompanied by adults

  • Pet visits are arranged with supervision and staff notification

  • Private rooms are available for families of actively dying patients

  • Curtain privacy in semi-private rooms for intimate family moments

  • All medical equipment, medications, and necessary medical supplies

Criteria for Inpatient Unit Admission

Hospice care continues as long as medically necessary. The Inpatient Unit typically serves as a short-term stay (usually 3-7 days) for symptom management, family respite, or end-of-life care.  This level of care is covered under Medicare Hospice Benefit and other insurance plans when the criteria are met. Should you have questions about the criteria, we’re happy to help.

Uncontrolled Pain or Symptoms

Patients experiencing acute or unrelieved symptoms that cannot be managed at home:

  • Severe or escalating pain requiring intensive management

  • Intractable nausea or vomiting affecting quality of life

  • Severe agitation or delirium requiring specialized care

  • Uncontrolled respiratory distress or dyspnea

  • Complicated wounds or bleeding requiring skilled interventions

Imminent Death Requiring Skilled Support

  • Rapid decline in overall condition

  • Multi-system failure requiring intensive monitoring

  • Families unable to cope with care needs at home during the final hours or days

Complex Medication or Treatment Needs

  • Frequent IV medications for pain management or seizure control

  • Medication titration requiring close professional monitoring

  • Need for advanced nursing assessments multiple times daily

Respite Care (Short-Term)

  • The family or primary caregiver requires brief rest (typically up to 5 days, as covered by Medicare)

  • Patient doesn't require crisis-level care, but admission provides essential caregiver relief

Navigating Our Process

Logistics are the last thing you should worry about. We've streamlined our referral process to ensure rapid response when patients need inpatient hospice care.

Referral Received

APEX Hospice Central Intake processes your referral.

Required Documentation

  • Patient facesheet
  • Current history and physical examination
  • Physician's order to evaluate and treat for hospice care

Initial Contact

APEX account executive reaches out to meet the patient and family.

Professional Assessment

Hospice RN visits patient at current location to:

  • Complete comprehensive patient assessment
  • Review case with Hospice Medical Director for approval

Coordination

If approved for IPU admission, an APEX Account Executive coordinates discharge planning and transfer logistics.

Clinical Handoff

The hospital provides complete clinical handoff to the IPU nursing team.
