Palliative Heart Failure Care — 4th Consecutive Year Palliative Heart Failure Certification

We are honored to share a major quality milestone: APEX successfully achieved our 4th year of Palliative Heart Failure certification with no deficiencies, aligned with our patient quality outcome matrix.
This continued success reflects the strength of our clinical standards, our interdisciplinary team coordination, and the measurable quality oversight we use to keep care consistent and reliable.
Earning this certification year after year — without deficiencies — signals that our program is not only clinically strong, but also operationally disciplined.
It demonstrates that APEX has repeatable systems in place to support complex cardiac patients with serious illness needs, including consistent assessments, responsive symptom management, and clear care planning that stays aligned with patient goals.

HCCI Innovation Award for CCM — Building Next-Level Chronic Care Expertise
We are proud to share a meaningful milestone: Apex received the HCCI Innovation Award contract for Chronic Care Management (CCM) and our team completed CCM training with expert consultants of HCCI, further strengthening our role as healthcare experts in managing complex chronic patients at home.

Chronic illness is rarely “one diagnosis”. Many of the patients you serve are navigating multiple conditions, medication complexity, symptom flare-ups, and caregiver strain — often leading to avoidable ED visits, readmissions, and fragmented follow-up.
Our enhanced CCM capability allows APEX to provide more structured, proactive support between visits, helping patients stay stable and confident at home while reducing preventable hospitalizations.
400 Palliative Patients Served
This year, APEX provided care for more than 400 palliative patients.
With a continued focus on improving quality of life at home, strengthening self-management and caregiver support, while promoting care coordination that supports lower overall healthcare costs.
2026 COPD Palliative Care at Home Certification
We’re excited to share a major clinical growth commitment for 2026: Apex will pursue certification in COPD Palliative Care at Home, expanding our ability to serve patients with advanced respiratory disease through a structured, evidence-based model of support.
This is more than a credential — it’s a deliberate investment in stronger outcomes, better patient and caregiver confidence, and improved coordination across the continuum of care.
Community Awareness & Education: National Cancer Awareness Month

APEX hosted a series of awareness activities throughout the year, and we were especially grateful for the opportunity to celebrate National Cancer Awareness Month with meaningful outreach and education.
During this month, our team focused on strengthening community understanding and how early interventions can make a difference — through timely symptom relief, compassionate guidance, and practical caregiver education that helps patients remain comfortable and supported at home.
Centralized Intake — Faster, Simpler Referrals Across All Service Lines
We are pleased to share an operational enhancement implemented in 2025 that directly supports our referral partners.
APEX centralized our intake process, creating a single, streamlined pathway for referrals across any service line. This improvement was designed to reduce friction for referral sources, accelerate decision-making, and shorten the time from referral to admission — while improving consistency, communication, and continuity of care.
Referral teams are busy, and patients often need timely support. In a decentralized model, referrals can experience delays due to multiple points of contact, duplicated questions, or inconsistent handoffs.
Centralized intake ensures every referral is captured, processed, and tracked through one coordinated workflow, reducing delays and preventing missed opportunities for patients who need care now.
RAISING THE BAR — Hospice CAHPS & Star Ratings

Strengthening quality outcomes is a strategic priority at APEX — and in 2025, we sharpened that focus even further by elevating our performance in Hospice CAHPS and reinforcing agency-wide accountability around CMS quality and Star Rating performance.
In alignment with our quality scorecard approach, we’re using KPI-driven management review and continuous improvement to support strong performance in Medicare quality measures, including the Hospice Quality Reporting Program (HQRP).
Hospice CAHPS is more than a survey — it reflects how patients and families experience care: communication, responsiveness, emotional support, and confidence in the care plan.
Star Ratings and quality measures also influence partner confidence, patient choice, and system-wide trust.
For referral partners, strong CAHPS performance signals dependable transitions, consistent communication, and a care team that is responsive when needs change.
